In one of these many important roles, you'll join a unique team of professionals led by a physician with experience in hospice and palliative medicine. Working together, and with the medical community, families and caregivers, you'll be responsible for ensuring that every hospice patient is treated with the greatest respect and dignity.
Below are some of the careers options available with Evercare Hospice & Palliative Care.
Hospice Case Manager Position
In this critical role, you will first identify the physical, psychological, social and spiritual needs of patients and their families, then coordinate the appropriate plan of care. It's a rare combination of management skills and intuition that we look for at Evercare Hospice & Palliative Care. Other requirements include reliable transportation, an RN license in hospice and strong clinical and nursing skills.
Hospice Social Worker Position
As part of the interdisciplinary team, it will be your job to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit, and provide both the counseling and casework needed to meet these goals. A Master's degree in Social Work is required.
Hospice Certified Nursing Assistant Position
This isn't just a position; it's a calling. You're in hospice because it's where patients need you the most. It's your heart and skilled hands that make the difference, and they're what we're looking for at Evercare Hospice & Palliative Care. You must be a graduate of an accredited CNA program and have at least one year of experience as a CNA, preferably in hospice, home health or long-term care.
Spiritual Health Position
Those in our Spiritual Health roles have the honor of providing patients and families spiritual support and comfort in this most difficult stage of life and during the year following the family's loss. A Master's degree from an accredited Seminary School of Theology is essential, along with two years of experience as a pastor or chaplain in a health care setting.
Hospice Physician
Evercare Hospice & Palliative Care is a physician-driven organization where you can make a difference. Not only will you be actively involved with patient care; you'll also be part of a local leadership team and a national Medical Director's team. You can shape the future of hospice care across the county. On a day-to-day basis, patient care will be your priority, and with a Medical Director-to-patient ratio of approximately 1 to every 100 patients, you'll really have the time to get to know your patients. Our physicians experience a broad range of care, from hospice to palliative, with no patient being turned away. To meet these challenges, we need physicians who are board certified in hospice and looking for a full-time hospice specialty.
